Learn more at pvhealthtrust.org Fostering health, wellness, and a full quality of life for all 85 Neilson Street, Watsonville, CA 95076 (831) 726-4257 Save the Date: CHT Health Fair Friday, October 1, 2021 The Hemophilia Foundation of Northern California serves the needs of people impacted by bleeding disorders through enhancing quality of life by providing support, education, outreach, advocacy and research through our affiliated national foundations. For 23 years, the Community Health Trust of Pajaro Valley (CHT) has enjoyed serving the community by providing much-needed diabetes education and support programs. We aim to con- tinue growing our impact while fostering our vision of an equitable Pajaro Valley with health, well- ness, and a full quality of life for all. The Diabetes Health Center (DHC) is CHT's largest program, aiming to bridge access to proper healthcare for those with prediabetes and diabetes. Our DHC works to pro- vide patients with diabe- tes education to self-man- age their diabetes and empower them to lead a healthier lifestyle. From Gestational Diabetes support to those newly di- agnosed with prediabetes, we see patients of all ages, from all backgrounds. We are also expanding our services to provide free wellness screenings at our El Mercado farmers' market. Another challenge facing our community is food insecurity and access to healthy, fresh food. The pandemic heightened these issues and created stress for local families. CHT has made it a priority to address this issue and is working to alleviate this problem through the launch of El Mercado, our weekly local farmers' market in Ramsay Park. Every Tuesday, communi- ty members get access to local organic produce and a selection of prepared hot meals. El Mercado also serves as a redemption site for our VeggieRX program; qualifying par- ticipants receive $20 food vouchers redeemable for free fresh produce. Another redemption site for VeggieRx is El Mini Mercado, housed at our DHC. This in-house food pantry also helps supply other local pantries in the area with produce donat- ed from generous local sponsors. Since opening earlier this year, El Mini Mercado has already dis- tributed over 6,000 lb. of veggies to local families. Thanks to our partner- ships with other organiza- tions and businesses, we can maximize our reach and spread the wealth of community resources.
