Resource Anesthesiology Association of California, a Medical Corporation ("RAA of CA") is a covered entity that performs anesthesiology services through a professional services agreement with Dignity Health, d/b/a Mercy Hospital Downtown and d/b/a Mercy Hospital Southwest. On July 8, 2021, a RAA of CA employee's laptop was stolen from their vehicle. RAA of CA notified law enforcement and filed a police report but was unable to recover the stolen laptop. RAA of CA investigated the known contents of the stolen laptop to determine what patient information may have been present on the laptop. Our investigation confirmed that patient information was present on the laptop, including the patient's name, address, date of birth, provider name, date of service, diagnosis and treatment information, health insurance information and other information related to patient medical care. If you are the individual listed on an impacted patient's insurance policy, then your name, date of birth, and health insurance number may have been present as well. Social Security numbers, financial account numbers, and other government IDs were not present in the data on the laptop. RAA of CA has issued notification letters to those who were impacted, but not all addresses were available, so we are also posting notice here. RAA of CA is not aware of any misuse of information, but we are offering, at no cost to the affected individuals, identity theft protection services through IDX. We are in the process of making sure all health professionals' laptops are encrypted. We are also retraining staff and reviewing policies and procedures related to patient confidentiality. Our patients' trust is a top priority for RAA of CA, and we sincerely apologize for any concern this has caused. We respect the privacy and confidentiality of all information entrusted to us, and we will remain vigilant in our efforts to safeguard and protect all information within our control. To determine whether you were affected by this incident, please call please call 1-833-992-4006 Monday through Friday from 6 am - 6 pm Pacific Time.
