Health Projects Center By John Beleutz, Health Projects Center Health Projects Center (HPC) is a Santa Cruz based non-profit organiza- tion dedicated to creating a community where people age at home with dignity and good health and where people receive supportive health and social services in the least restrictive en- vironment possible. HPC programs are delivered in both English and Spanish for older adults, family caregivers and health workers in the Monterey Bay Region. HPC strives to help peo- ple age safely at home by: 1. Managing health care and supportive services for medically frail, low-in- come elders. 2. Supporting family care- givers to provide quality care for their loved ones and their own physical and mental health. 3. Strengthening the health care workforce to effec- tively address geriatrics and in-home care manage- ment. HPC supports over 1,300 local families annually to live and age at home at a fraction of the cost of a long-term care institu- tion. The average annual rate for skilled nursing facilities in California is $112,044 per year. If HPC's combined activities keep only 30 people out of long-term care institutions per year, HPC will save families and government over $3.3 million per year. This is equivalent to the full annual budget for HPC. HPC programs have become even more import- ant during the time of COVID-19. People are able to remain safe at home with the help of the services HPC can provide. Over 40% of COVID-19 deaths in California have occurred in assisted living facilities. Keeping people in their home during these unpredictable times is an important aspect of HPC's mission to keeping the community safe. HPC recently partnered with the County of Santa Cruz and the Medical Reserve Corps to provide mobile in-home COVID-19 vaccination to 60 high-risk home-bound elders. Care Management HPC's Multipurpose Senior Services Program (MSSP) manages health care and supportive ser- vices for medically frail, low-income elders so they can stay safely in their home. A team of nurses and so- cial workers provide each MSSP participant with a complete health and psy- chosocial assessment to determine needed services. The team then works with the MSSP participant, their physician, family and others to develop an individualized care plan. MSSP services are free and include care manage- ment, minor home repair, transportation, supplemen- tal in-home care, respite for caregivers, home de- livered meals, communi- cation devices and money management. To be eligible for MSSP, participants must have sig- nificant medical issues, be 65 years of age or older, have appropriate Medi-Cal eligibility and have a will- ingness to participate in an individualized care plan. Family Caregiver Sup- port HPC's Del Mar Caregiv- er Resource Center (CRC) supports family caregivers to provide quality care to their loved ones and take better care of their own physical and mental health. CRC is part of a state-wide network pro- viding supportive services to family caregivers of those living with cognitive impairments. Family consultants work with clients to increase caregiving knowledge and skills, to build caregiver confidence, to strengthen emotional well-being and coping abilities and to bridge social connections linking families to com- munity services. Through CRC, caregivers are provided services such as personalized caregiver assessments, one-on- one family consultation services, consultations with an elder law attorney, support groups, work- shops, conferences, online education and paid respite care. CRC provides free and low-cost services to indi- viduals responsible for the care of a family member living with an adult-onset neurological disorder. All families are welcome, in- cluding extended kinship networks and families of choice. Individuals are eligible for services regardless of income and resident status. Respite services include a sliding scale co-pay fee. All other Del Mar CRC services are at no cost. Caregivers can receive support at https://delmar. cacrc.org/login and by joining CareNav. By joining CareNav, caregiv- ers will be asked a brief set of questions that lead to a personal dashboard loaded with information that matches individual caregiving needs, such as: Fact and tip sheets, vid- eos, online classes and support Caregiving resources nationally and by state Access to a skilled Family Consultant Dashboard to track infor- mation and find support Health Care Workforce HPC's Central Coast Area Health Education Center (AHEC) works to strengthen and support the health care workforce to effectively address geriatrics and in home care management and to improve access to and the quality of health care for the medically under- served in the Monterey Bay Region. AHEC is a national network of cen- ters that work to educate and train health students and professionals serving underserved communities. Central Coast AHEC activities educate family practice medical resi- dents, health professions students and health care workers on geriatrics and community-based care. AHEC works in partner- ship with the UCSF's Natividad Medical Center Family Residency Program and Cabrillo Col- lege's Nursing and Dental Hygiene programs. AHEC also provides training to In-Home Supportive Service workers (in both English and Spanish) em- ployed through Santa Cruz and Monterey Counties. AHEC builds communi- ty-academic partnerships for health education and health workforce training.
